HOSPICE CARE

We help patients and families make memories and enjoy their lives fully. Our doctors and nurses specialize in managing pain and symptoms that are unique to life-limiting illnesses. Other medical professionals are not trained to do this. We come to you. Patients can stay at home longer because we make house calls, we’re on call 24/7, and we deliver any needed medical equipment. We help for several months.

BUTTERFLY PROGRAM

To better serve children, Houston Hospice initiated The Butterfly Program; providing special services for children with life-limiting conditions who are aged 18 and younger. This program includes perinatal hospice for families who have received a prenatal diagnosis indicating their baby will likely have a limited lifespan.

BEREAVEMENT SUPPORT

We are committed to offering a variety of bereavement services to individuals, with special emphasis on providing family bereavement support. The goal is to help the patient’s family and other loved ones cope with grief and loss issues. Bereavement services begin with the initial assessment and an individualized care plan and continue 13 months following the death of the patient. The care plan may include grief educational group meetings, informational literature, correspondence, phone calls and our special seminars around the holidays.

EMPLOYEE PARTNERS

Our quality of care is demonstrated by our staff and the high level of care that each of them provide to patients and families. Our physicians are experienced and board-certified and many of our care team, from nurses, social workers, hospice aides, to chaplains, have advanced certification in hospice and palliative care. Houston Hospice physicians are an integral part of the care team. Our team is different because we are a nonprofit organization. Our multidisciplinary teams work together, along with our patient’s primary physician to create a customized, all encompassing care plan.

NONPROFIT HOSPICE MODEL

Houston Hospice is the only nonprofit hospice in Houston. Because of community support, we’re able to offer more supplementary services to patients and families. Patients have a choice and they can request Houston Hospice. We are the hospice where mission and the community meet. We have deep community relationships with local health systems, schools, government agencies, churches and clergy, community organizations and other partners. Our bereavement support and volunteer involvement also separate us for for-profit hospices. Houston Hospice care is covered by Medicare, Medicaid and many private insurance plans.

We share information, resources, and ideas with a diverse array of community organizations throughout Houston and tailor to each organization’s unique needs. Our mission and the community meet to increase awareness among the general public about hospice as a valuable service.
