Houston Hospice supports veterans by providing the highest level of specialized care focused on each individual’s unique needs and the needs of their families. Our interdisciplinary team collaborates with each veteran’s primary physician and family members to create a customized, veteran-centric care plan. Each team consists of the following hospice specialists who honor veterans for their service and take their experiences into consideration when planning hospice care:

  • Physician
  • Nurse
  • Hospice Aide
  • Chaplain
  • Social Worker
  • Bereavement Specialist
  • Volunteer

Houston Hospice, in partnership with We Honor Veterans (WHV), a program of the National Alliance for Care at Home, and in collaboration with the U.S. Department of Veterans Affairs (VA) has developed a specialized hospice care plan for the increasing number of veterans nearing end of life. To meet the needs of this growing population, Houston Hospice’s Veterans Program, which focuses on education, advocacy and mentorship, has reached WHV Level 4 status.
